No login assistance.

Where to Find It
The Patient Link QR Code is available across multiple areas of the Mend Dashboard (Portal):
- Appointment Queue
- Upcoming Appointments
- Current Appointments
- Outstanding Assessments
It is also available within the Fusion Extension:
- Appointment Queue
- Actions Menu on the Patient Page
This ensures teams can access it wherever they are already working.
How It Works on the Mend Portal
Step 1: Open the Actions Menu
If a patient has forms assigned that are associated with an appointment:
- Click the three dots (meatballs menu) next to the appointment or form.
- Select “Patient Link QR Code.”
Step 2: Display the QR Code Securely
Once selected:
- A QR code modal appears on screen.
- All other dashboard information is hidden.
- No PHI or sensitive patient data remains visible in the background.
- The QR code can safely be presented on:
- A tablet
- A desktop monitor
- A mobile device
This design ensures front desk staff or providers can turn the screen toward a patient without risking exposure of other patient information.
Step 3: Patient Scans the QR Code
The patient scans the QR code using their phone camera, or a device that is provided to them, like an organization-owned tablet.
From there:
- If Check-In is enabled → The patient is taken to the check-in screen to complete required forms.
- If Check-In is not enabled → The patient is taken directly to their assigned form(s).
Regardless of your organization’s check-in configuration, the patient is directed to complete their required documentation before their visit.
How It Works in the Fusion Extension
For teams working inside the Mend Fusion Extension:
- Navigate to the Appointment Queue or the Patient Page.
- Open the Actions Menu.
- Select “Show QR Code.”
The QR code modal will appear directly within the extension overlay.
This keeps the workflow entirely within the provider’s EHR view, without toggling systems.
