This article seeks to provide patients with information on the SMART form that comes with a healthcare organization opting into Mend's Inclusivity features. This form will allow you to enter preferred names, pronouns, and other information that will allow your healthcare provider to better get to know you and facilitate your care.
Please note that if you do not wish to answer the SMART form, you will be provided an option to answer with "Prefer not to say".
These demographics can be updated in the following ways:
- Your patient profile
- Staff assigned SMART forms
- Through account creation.
You can access your patient profile to change your demographics yourself, you can do so by searching yourself on your PATIENT tab and clicking Actions > Edit

Your form will ask you to add or update the following information.
Note: An * means this is a required question or could be a required question depending on your healthcare organization
- Legal Name*
- Name You Go By
- Date of Birth*
- Gender Assigned at Birth*
- Gender Identity
- Pronouns
- Contact Information (Email only)*
- Address Information*
- Additional Information (Language and Time zone only)*

- This form doesn't have my pronouns of choice
For custom pronouns, you can use the "Other" selection. "Other" selections will show on your end but not on the Staff+ end. If you would like to have a selection that appears for both sides, you'll need to contact your healthcare organization so they can add your custom pronoun to their official list.
- I entered something wrong on the form or I need to change something on my form
To make a correction on a form you already submitted it, you can contact your healthcare organization so they can make the corrections for you or assign you a new SMART form for you to complete. You can also search your account with the Patient tab, then click Actions > Edit to make any adjustments you need.
- I'm getting an error when completing the form/I can't submit the form!
Try refreshing your page, and double check that all required questions (marked with a red asterisk) are filled out, then try again. If the issue persists, feel free to click the "Tech Support" button in the top right of your Mend page to connect with one of our agents for assistance!
